*If you are registering as an exhibitor, please enter the name of the individual attending the conference. If there is someone else we should contact about the booth information, there is space to provide that name. If more than one person is traveling to the conference, any other individuals will need to register separately.

Cancellation Policy: All cancellations must be submitted in writing by e-mail to iccdu-planning@ems.psu.edu or by fax to 814-863-7432 by Monday, June 3, 2013 for a full refund, less a $50 administration fee. Cancellations after June 3 will not be refunded. In lieu of cancelling you may transfer your registration to another person without penalty.
